Appointment / Consultation Request Form
*If you have pictures to share with me (such as examples of images you like, or an existing tattoo for a cover-up request) email is best. Please provide the following information in your email:
Name / Phone Number Include your name and a phone number where you can be easily reached. Tell me when is best to call, if necessary. Optional: include a phonetic spelling of your name (if you find that people often get it wrong) and your pronouns.
What's your idea? What is the tattoo you're looking to get (in as much detail as possible)? If you're not totally sure what the image will be, but you have a feeling/concept/theme you're going for, tell me your thoughts and we'll go from there.
Where is it going? Inner bicep? Left shoulder blade? Top of right thigh? Be as specific as possible. Again, if you're not sure or want suggestions, let me know!
What size? The above questions will mostly determine what size your tattoo needs to be, but if you have an idea of how much space you want the tattoo to take up, tell me.
Are you inquiring about a cover-up? Be sure to send pictures of the existing tattoo, including the available space around it. My general approach is that cover-ups need to be about three times the size of existing work to allow for appropriate black shading, negative space, and detail. If you have had laser treatments to lighten the original work, the 3X rule may not apply. We can determine that as we move forward. I don't accept every cover-up request. If you hope for the cover-up, but you're open to the same tattoo idea on fresh skin, let me know.
